Companies need both business and narcotic authorisations from the Chief Pharmaceutical Inspector; only research institutions may cultivate, and just one permit has been granted. Marketing authorisation for raw cannabis products is lengthy — often over two years — and products are processed in pharmacies.
Any doctor permitted to prescribe narcotics can prescribe medical cannabis; in practice neurologists, oncologists and anaesthesiologists dominate, alongside specialised and telehealth clinics. There is no official list of indications — it is left to the prescriber.
There is no reimbursement; patients pay out of pocket. Pharmacy-dispensed volumes rose sharply through 2023 on the back of new products, stronger supply chains, lower prices and telehealth access.
The market is overwhelmingly flower with a smaller share of extracts; most flower originates from Portugal and the remainder from Canada, typically packaged in Germany before export to Poland.
Poland is import-reliant with no active domestic commercial cultivation; early shortages eased as more companies obtained import permits and product authorisations.
