Czech Republic has had a medical cannabis framework since 2013 and decriminalised personal possession in 2021. The country made its first cannabis exports in 2024 to Germany, with SensiQure and Lagom Pharmatech leading the supply chain.
The Czech Republic first legalised medical cannabis in 2013 through Act No. 50/2013, allowing patients with specific conditions to receive treatment. Subsequent decrees by the Ministry of Health have expanded access and commercial frameworks significantly.
In January 2022, an amendment to the Addictive Substances Act eliminated the tender process for domestic cultivation, allowing companies to apply directly for a cultivation licence. This spurred significant growth in commercial cultivation and an export market.
August 2022: Decree No. 235/2022 published, establishing rules and requirements for cultivation and processing (growing practices, security and storage, recording, harvesting).
Commercial operators must hold specific permits for handling addictive substances plus relevant activity authorisations (import, distribution, cultivation, etc.), issued by SÚKL or the Ministry of Health.
Who can prescribe: General practitioners for chronic pain (from April 2025). All other conditions require a specialist with one of the following specialisations: Clinical or Radiation Oncology, Neurology, Ophthalmology, Psychiatry, Palliative medicine, Pain Management, Rheumatology, Dermatovenerology, Orthopaedics, Infectious medicine, Internal medicine, or Geriatrics.

Reimbursement: Public health insurers cover 90% of costs for up to 30 grams per month. Quantities above 30g are paid out-of-pocket.
Patient growth: Peak increases in 2020 (+2,500 patients) and 2024 (+2,722 patients). Annual growth rate approximately 10–15%. In 2024, pharmacies dispensed 318 kg; H1 2025 already 170 kg dispensed.
Price cap for domestic cannabis (set 2022): CZK 143.75 (~€5.70) per gram. Retail flower prices: €5–8 per gram before VAT. Extract cap: CZK 1,006.25 (~€40) per gram before VAT.
65 flower products and 5 extract products identified on market. Majority (41) are high-THC; 9 balanced; 15 CBD-dominant.
Market is approximately 90% flower-dominated. Extracts introduced 2022, growing from 8% to 14% of products by end of 2024.
Key suppliers: Aurora (largest portfolio, mostly imported from Canada). Imports also originate from Denmark, Portugal, and Lesotho (via Germany). Domestic EU-GMP cultivators expanding: Lagom Pharmatech, SensiQure, Zenplanto, Motagon.
First medical cannabis exports to Germany occurred in 2024 — over 1,300 kg exported over the year.
Exporters include: SensiQure (via Phytopemp and Nimbus Health), Lagom Pharmatech, and Czech Medical Herbs (Drapalin Pharmaceuticals).
Export volumes ramped up rapidly through all four quarters of 2024.
The Czech Republic remains import-reliant for patient supply but is rapidly growing its commercial cultivation and export capabilities.
